•Only add once per order.
If you're ordering multiple tags, we'll add the Owner Alert hotline to all of your new tags. If there's a tag you don't want the Owner Alert hotline added to, just let us know by adding a note in the Special Instructions box when checking out.

•A purchase of at least one ID tag is required.
We only add the Owner Alert 800# hotline to tags. We do not add the 800# hotline to personalized collars or leashes.

•If you'd like to add more phone numbers to your current list on file, add them below; or leave blank to keep what you already have on file.
